Functional Medicine Provider (MD, DO, PA, NP, DC or ND)

Bring a root-cause, whole-person lens to a nervous-system-first practice, working alongside our chiropractic team to help families get answers without spending $$$ on hefty supplement regimens but have real clear pathways to healing.

Our patients come to us after hearing ‘your labs are normal’ one too many times. You’ll help them find out why they still don’t feel well. You’ll work alongside our chiropractors to build integrated care plans for families dealing with fatigue, hormones, thyroid, gut health, fertility, PANS/PANDAS, POTS/dysautonomia, anxiety and more. Ideally you would want to do a sort of direct primary with families as well.

What you’ll do:

  • Run functional consults and follow-ups for adults, moms and kids
  • Order and interpret functional lab testing, and build clear, doable and simple- one system focused care plans
  • Collaborate with our doctors on shared patients and integrated care
  • Educate patients and the community through workshops, webinars and content
  • Help shape and grow a new functional medicine service line at Transform

You might be a fit if you:

  • Hold an active license as an MD, DO, PA, NP, DC or ND in Minnesota
  • Have training or experience in functional, integrative or root-cause medicine
  • Love teaching and explaining the ‘why’ to patients in plain language

You’re the operator who turns vision into a practice that runs smoothly every day. You’ll lead our support team, own our systems and track the numbers that tell us how we’re doing, all while elevating the patient experience.

What you’ll do:

  • Lead daily huddles, team scorecards and weekly accountability
  • Hire, train and develop our care team alongside our lead doctor
  • Build and maintain SOPs for scheduling, billing, patient flow and follow-up
  • Track key practice metrics (collections, new patients, retention) and drive improvement
  • Partner with the lead doctor and marketing team on growth, marketing and community presence

You set the tone for every family who walks through our doors. You’re the friendly voice on the phone, the calm in a busy schedule, and the person who makes sure nobody falls through the cracks.

What you’ll do:

  • Greet patients and families, check them in and out, and keep the day flowing
  • Manage the schedule, phones, texts and email inquiries
  • Walk new patients through their first visits, financial options and next steps
  • Follow up with patients to keep their care on track
  • Help create an office atmosphere that feels like home

You might be a fit if you:

  • Are highly organized, warm and steady under pressure
  • Communicate clearly and kindly, in person and in writing
  • Are comfortable learning scheduling software and a CRM
  • Have front desk, hospitality or customer service experience (healthcare a plus, not required)

Our INSiGHT scans show patients what’s happening in their nervous system, and you’re the person who runs them. Your main job is to create an excellent patient experience and make sure every patient’s progress is checked and celebrated.

What you’ll do:

  • Perform INSiGHT nervous system scans for adults, kids and babies
  • Own the quality checkpoints that track each patient’s progress
  • Keep scan equipment, software and systems running smoothly
  • Create a warm, fun experience, especially for our littlest patients
  • Support the front desk and patient flow as needed

  • Are detail-oriented and love people
  • Are great with kids and nervous parents
  • Are comfortable with technology and following protocols
  • Are interested in healthcare, wellness or chiropractic (we’ll train you on scanning)

FAQ

Do I need chiropractic or healthcare experience?

Not for every role. For Front Desk and Scan Tech, we hire for heart and train for skill. The Functional Medicine Provider and Director of Operations roles require the experience listed in each description.

I’m a current patient. Can I still apply?

Absolutely. Many great team members start out as patients who already love what we do.

Why do you ask for a video?
